    Health New Zealand's had to apologise to the family of an elderly man who died less than an hour after discharge from Taranaki Base Hospital


    The Aged Care Commissioner found Health New Zealand breached the Health Code, and didn't provide an appropriate standard of care.

    The man, in his 80s, had breathing issues and abdominal pain.

    He was sent home after five days in hospital with medication, still in pain.

    His daughter says upon arriving, he struggled getting up three flights of stairs, and died five minutes later.

    Health NZ Taranaki now has a trained senior nurse to act as complex discharge coordinator.
