    A medical centre GP registrar failed to establish a woman was pregnant before inserting a Jadelle contraceptive device


    A report's found the woman's rights were breached, after her realisation she was pregnant when it was fitted.

    The Jadelle was removed a month later, and the woman was sent an apology letter.

    Health and Disability Commissioner Rose Wall says the case highlights the importance of reliable services in contraceptive consultations.

    She concluded the GP registrar didn't clearly establish whether the woman could be pregnant, or offer her a test.

    They've made changes to training and documentation practices since.
