A woman believes she blacked out while driving after the wrong insulin pen was incorrectly dispensed

The pharmacy had supplied a seven day dosage of a fast-release pen in 2021 - which she thought was her usual slow release medication.
But after experiencing blurred vision, dizziness and fainting spells, she checked the medication and realised the discrepancy.
Deputy Health and Disability Commissioner Dr Vanessa Caldwell's says while the pharmacy accepted responsibility and made improvements - it failed in its responsibility to provide reasonable care.
