43-year-old Taranaki Fuimaona was admitted to Auckland City Hospital in June 2021 complaining of severe abdominal pain and was placed into an induced coma.
Medical staff treating the Head Hunters member found meth in his underwear and called police.
The Independent Police Conduct Authority found the following day, officers incorrectly completed an evaluation form and monitored him every two hours, instead of 10 to 15 minutes.
The authority says, over the course of five hours, Fuimaono's condition deteriorated until he was found unresponsive and later died in hospital.
