Several recommendations from a coroner, in light of a Takaka truck driver's workplace death

62-year-old Phillip Holt died in September 2021, pinned between his runaway truck and another vehicle.
The hand-brake wasn't on when Holt got out of his vehicle, and it rolled forward.
The alarm signalling \the brake was off, did not function.
Coroner Mary-Anne Borrowdale's recommended Solly's Freight alter its pre-start inspection requirements, to have a driver check every onboard safety system.
She's also called for NZTA to amend its heavy vehicle inspection manual.
